In order to monitor the equality element of our organisation’s policies and practices effectively, we would be grateful if you would complete this form. Sport England require us to keep both our Staff and Board equality profile up to date and monitoring is helpful to influence the development of future policies and programmes. Most questions relate to the protected characteristics covered by present equality legislation, known as the Equality Act 2010. This year we have introduced a new question about neurodiversity to support greater neuroinclusion in the workplace. The sensitive personal data received from this questionnaire will be stored, processed and monitored by authorised personnel of Yorkshire Sport Foundation in accordance with the Data Protection Act 2018 and UK GDPR. Results will only be produced in a depersonalised and aggregated form. If a question is not answered the response will be shown as 'prefer not to say'. Completion of all questions within this form is optional, but the more information we have, the more effectively we can operate as an organisation.
